Contest Rules & Regulations
In order to participate in the NFOSD T-Shirt Contest, you hereby agree to the following rules and regulations:
Eligibility
The contest is open to anyone regardless of swallowing disorder status – there are no age restrictions.
Design
Be prepared to upload your design in the following format: .jpeg or PDF. Your design does not need to include a t-shirt outline.
Any and all text included in the design should be in English.
The winning design will be selected by the NFOSD and printed on the front of the official Dysphagia Awareness Month T-shirt. The color of the conference shirt this year is TBD.
The NFOSD reserves the right to use any and all designs submitted for future marketing purposes.
No reproduced images will be accepted. Your submission must be original artwork. Any copyrighted images without explicit permission for use by the original illustrator will be automatically disqualified.
There is no limit to the number of entries per participant during the entry period.
If your submission needs to be reproduced to ensure the highest quality of printing, we will do our best to recreate the most accurate design.
Deadline for submission: April 20, 2020 (midnight ET)
You must complete the entire application. Entries will not be accepted if application is incomplete or sent separately from the online form. Please contact info@nfosd.com for any technical issues with submitting your design.
Criteria for Selecting the Winning Design
The winning design will be determined by the NFOSD Board based on the creativity of the design and how well it incorporates the essence of the National Foundation of Swallowing Disorders and the dysphagia community.
Selection of winner: May 1, 2020
Winner will be notified via email.
Prize
The winner will receive the following:
- One T-shirt
- $100 Visa Gift Card
- Recognition on the NFOSD website, emails and social media.
Disclaimer
The submission form must be filled out completely and truthfully. If you fail to complete the form, give any false information or fail to provide any required information, you may be disqualified from the competition.
All decisions are made by the NFOSD and are final.
If winner cannot be contacted within two weeks of selection, prize may be forfeited, however the design may still be used.
